There are different DSA application forms for the different funding bodies. Please download the appropriate form as outlined below.
For students except those expecting to receive a bursary from the NHS, Department of Health or a research council:
- Complete the short version of the DSA1 form (pdf) if you have already completed an application for the Student Loans, grants and bursaries available to full-time students (or you are intending to do so).
- Otherwise, you should use the full version of the DSA1 form (pdf). You will also need this to complete this version if you are a part-time or postgraduate student, or if you are studying your course by distance learning.
- You can also download these notes to help you complete the DSA1 forms (pdf).
For students expecting to receive a bursary from the NHS or Department of Health (e.g. Occupational Therapy and Physiotherapy courses) complete the NHS Disabled Students' Allowances application form (pdf).
For students studying a postgraduate Social Work course complete the Disabled Students' Allowances application pack.
