This research evaluates the implementation and impact of recent policy reforms in Catalonia aimed at integrating social care and primary healthcare systems, with a particular focus on the requirement for nursing homes to be formally integrated with primary care services.
The study examines whether these reforms improve care coordination, quality of care, and health outcomes for vulnerable populations, particularly older adults with complex needs.
The impact is underpinned by a collaborative research project evaluating the integration of social care and primary healthcare in Catalonia following recent legislative reforms requiring nursing homes to be formally integrated with primary care services. The project is conducted in partnership with the Institut Català de la Salut (ICS) and the Catalan Social Care Department, providing direct access to system-level data and policy processes.
